You could compare it to the fact that you could be the most popular person in high school, your job,etc. and still feel completely and utterly alone. The song is a very emotional song for Michael because it's about when he felt lonely, for a very long time, he was very miserable, that's what the song is about! When he is on stage he has all of his fans but when he is offstage he is lonely, because he is a big star he doesn't have many "real" friends!
